How can I seamlessly transition this flooring blend across my three entrances to prevent it from becoming a trip hazard?

Is the tile going on top of the hardwood floors? If it is, it won’t work. The hardwood should have been taken out for a correct installation.
Transition strips are an easy solution for stepping down. It may take some time to decide on the color, but you can either match the tile or match the floor.
Is the tile placed over the hardwood floors? If so, there may be a risk of cracking. The best solution would be to install a transition strip, typically made of wood, and stain it to match the hardwood. It’s recommended to use a wider strip, but in this situation a 4 inch strip should do the job. This will create a smooth transition that resembles a small ramp.
You can try cutting a custom transition on a table saw and then stain it to match. If you’re new to this, I suggest practicing on some scrap wood first before using a quality piece. I often create custom transition pieces during remodels or additions.
