How can I repair a gap between the foundation and frame of my backdoor that is allowing moisture in, as well as the nearby cracked foundation?

Hey, it seems like the foundation is starting to separate from the frame of the backdoor, creating a gap for moisture to get in. Any ideas on how to take care of this? I also noticed a cracked foundation nearby in a photo.

I’d recommend having a professional take a look at that. Horizontal cracks can be quite concerning.
